NOTICE

The Embassy of India, Kyiv is participating in a centralised project operated by Ministry of External Affairs (CPV division) for issuing machine readable passports with effect from August 26, 2007. Passports would thereafter be printed in India and couriered to the Embassy. Once the system becomes fully operational it would require around fifteen days * after processing of application to issue the passport by the Embassy to the applicant. Please bear with us for any inconvenience caused during this transitional phase.


Passport & Consular Services

Welcome to the passport services of the Embassy of India, Kyiv. Applications for Passport and consular services can be made at the Consular Section of the Embassy, which is open from 09:30 hours to 11:30 hours on all Monday, Wednesday and Friday. Passports, after completion of the service requested, can be collected from 17:00 and 17:30 hours on the date indicated. Fees for passport services are accepted at the Embassy in cash in US dollars. Application forms and suggested formats for affidavits etc. can be downloaded from this website or obtained from the Consular counter at the Embassy. New Passport on Final Expiry
Application form (Form 1) duly filled and signed to be submitted along with 4 photographs and the old passport along with a photocopy of the first and last page of the passport.
Fee: USD 40 for 36 page passport or USD 50 for 60 page jumbo passport
Applications for new passport can be submitted within one year before the final Expiry of the current passport.

    Time Taken: *
  • Normally fifteen days if the Original Passport (10 year validity) issuing authority is this Embassy.
  • If the passport was issued by any other authority, at least 40 days or before if clearance from concerned authorities is received.

Additional booklet for existing valid passports
Application form (Form 2) duly filled and signed to be submitted along with 4 photograph and the current passport
Request will be entertained only if all pages are exhausted.
Fee: USD 20 for 36 page passport or USD 25 for 60 page jumbo passport to be issued for remaining validity.
Time taken: as applicable for news passports

Duplicate Passport in lieu of lost/damaged passport
If you are on a short visit to Ukraine and have lost or damaged your original passport you can approach the Consular Section for either a Short validity passport (or an emergency certificate).
Application form (Form 3) duly filled and signed to be submitted along with photograph.

  • Supported by police report regarding loss of passport
  • Sworn Affidavit for duplicate passport.
  • Photocopy of the lost passport (if available). The application should be supported by any other document issued to the applicant in India - driving Licence, ration card, voters Identity Card, etc.
Fee: USD 125
Time taken: Till clearance is received from the authorities is concerned.

Emergency Certificate
Form for issue of Emergency certificate to be duly filled, signed and submitted along with 4 photographs.

  • Details of the flight and transiting country should be mentioned.
  • Photocopy of the passport held.
  • Any other document to establish Identity.
Fee: USD 5
Time taken: Only on establishing Identity.

Passports for Minors (< 18 years)
Original and photocopy of the Birth Certificate issued by the hospital/commune parents' passports in original and copies, application form (Form 1), duly filled in and signed, four recent passport size photographs, marriage certificate(if the name of the spouse is not endorsed on the respective passport).
Fee: USD 25 for 5 year validity passport of 36 pages.
Please ensure that column No.28 on page No.6 of the application form (declaration of parents/guardian if applicant is minor) is duly filled in and signed by both the parents.

Passports for Children of Indian origin born in Ukraine
Children's name cannot be entered on their parent's passport any more. For a child to be given an Indian passport his/her birth must have been registered with the Consular section of the Embassy within three months of birth. To apply for issue of a new passport to a child you must fill out

  • 4 photos of infants with eyes open
  • Photocopy of the passports of the parents.
  • Photocopy of the child's birth certificate issued by hospital (along with original)
Fee: USD 25 for a 36 page passport of 5 year validity

Change of Appearance
Application form (Form 1) duly filled and signed along with 4 photographs to be submitted along with:

Change of Address
Application form (Form 2) duly filled and signed along with 4 photograph to be submitted along with:

  • Passport.
Fee: USD 10

Change of Name
Application form (Form 1) duly filled and signed along with 4 photograph to be submitted along with:

  • Passport.
  • Cuttings of two leading newspaper (one in India and the other in Ukraine) carrying advertisement notifying the change of name and asking for any objection thereof to be reported to Embassy of India within 30 days of the publication.
  • Sworn Affidavit in the prescribed format
Fee: As applicable for new passport
Time taken: One week.

Change of maiden name upon marriage or re-marriage or divorce or death of spouse
Application form (Form 1) duly filled and signed along with 4 photograph to be submitted along with:

  • Passport.
  • Marriage certificate (or divorce deed or death certificate) duly attested by the Home Department of the State Government concerned and counter attested by the Ministry of External Affairs, CPV Division, Patiala House Annexe, Tilak marg, New Delhi.
Affidavit in the prescribe format joint photograph (in case of marriage or re-marriage)
Fee: As applicable for new passport
Time taken: One week.

Change of Signature
Application form (Form 1) duly filled and signed along with 4 photograph to be submitted along with:

  • Passport.
  • Sworn affidavit (General).
Fee: As applicable for new passport
Time taken: Normally 3 days

Change of any particular mentioned under lamination cover - Correction of spellings of names, Date of Birth etc.
Application form (Form 1 or Form 2) duly filled and signed along with 4 photographs to be submitted along with:

  • Passport.
  • Documentary proof to affect the change attested by the Home Department of the State Government concerned and counter attested by the Ministry of External affairs, CPV Division, Patiala House Annexe, Tilak Marg, New Delhi. Fee: USD 40 if new booklet is required to be released or USD 10 if an endorsement on the existing passport would suffice

    Deletion of spouse's name
    Application form (Form 2) duly filled and signed along with 4 photograph to be submitted along with:

    • Passport.
    • Court decree dissolving the marriage or death certificate duly attested by the Home Department of the State government concerned and counter attested by Ministry of External Affairs, CPV Division, Patial House Annexe,Tilak Marg, New Delhi.
    Fee: USD 10
    Time Taken: Normally 3 days

    Inclusion of spouse's name
    Application form (Form 2) duly filled and signed along with 2 photographs to be submitted along with:

    • Passport.
    • Marriage certificate, if from India should be attested by the Home Department of the State Government concerned and counter attested by the Ministry of External affairs, CPV Division, Patiala House Annexe, Tilak Marg, New Delhi.

    Emmigration Clearance Not Required
    Application form (Form 2) duly filled and signed along with 2 photographs to be submitted along with passport
    Diploma Certificate or proof of having stayed abroad for more than three years or proof of having paid Income Tax in India for the last three years. The Degree/Diploma Certificate should be attested by the Consular Section of the Ministry of External Affairs, New Delhi.

    Extension of validity of passport
    Application form (Form 2) duly filled and signed along with 4 photographs to be submitted along with:

    Ordinary passport (fresh or in lieu of an expired passport) of 36 pages. (No extra fee will be charged for renewals up to 10 years if initial validity is less) US$40.00

    Certificates and Attestations

    The consular section of the Embassy issues a number of certificates and performs notarial duties by way of attesting documents.
    They are as follows:

    General Guidelines

    1. To apply for any certificate, one has to fill "Form 2" form and submit it along with one photograph, appropriately pasted, and the passport.

    2. For a child born in Ukraine to be registered as an Indian citizen, the application must be made within three months of the child's birth on the prescribed form. . At least one of the child's parent's must be an Indian citizen at the time of applying. Along with the application please enclose passports of both parents and a full version birth certificate of the child. Photocopies of the birth certificate and of the first four and last two pages of the parent's passports are also required. The fee for this service is US$20. You also need to submit an affidavit stating that your child has not acquired Ukrainian nationality.

    3. For Police Clearance Certificate you are required to submit your passport along with photocopy of the first four and last two pages of your passport and a declaration as per standard format. The fee for this service is US$20.

    4. Sponsorship Certificate - If you wish to sponsor a person from India for a visit to the Ukraine and wish to apply for a sponsorship letter you need to submit the application in the prescribed form. You also need to submit your valid passport, proof of ownership of property if any, bank statements, salary slips or any other proof of income along with a photocopy. The application form must be signed by you in the presence of a Embassy official at Consular Counter. The fees for this service is US$10.

    5. To get any document attested by the Embassy, one needs to bring the document in original along with a photocopy, and valid passport. You will be required to sign the documents in the presence of Embassy officials at the service counter. Documents such as Marriage/Birth certificates which are in Ukrainian/Russian language and issued by the concerned authorities, should be submitted alongwith a self attested English translation for reference and record of the Embassy.

    6. Educational degrees and other documents issued in India and presented at the Consular section for attestations must have been first attested by the State Government concerned (Home Department, General Administration) and counter attested by Consular Section, Ministry of External Affairs, Patiala House Annexe, Tilak Marg, New Delhi.

    7. For attesting power of attorney, the person has to sign in the presence of the Consular officer. A photocopy of the power of attorney along with two photos of the executant and photocopy of the passport may also be brought.
